Complimentary 10-15 minute phone consultation.
An initial comprehensive assessment, will be conducted at our offices, in order to obtain a bio-pyscho-social-spiritual understanding of the client, thus, providing for an accurate and appropriate recommendation of treatment.
Prior to your first appointment:
- Please provide a copy of your insurance card and verify your benefits and co-pay.
- Please provide credit card information.
- Please complete intake documents, found in the link below.
Cancellation Policy: 24-hour notice required. Failure to do-so will lead to charges for the full cost of the service.
Please arrive 15 minutes early to your first session to complete intake documents, or provide your email address to have intake documents sent prior to your session.
